STEP 1: REVIEW THE DOCUMENTATION
- Make sure that the need for iLevel was specifically addressed.
- Facilitate independent transfers
- Increases reach to help with your activities of daily living in the home,etc.
- Does it include the following?
- Description of your current level of function without iLevel
- Identifies how iLevel will improve function
- Your goals for use of the power seat elevation system
STEP 2: MAKE SURE THE DOCUMENTATION IS ORGANIZED AND EASY TO FOLLOW.
- Highlight any important topics or areas that may have been overlooked in the initial determination.
STEP 3: CALL OR MEET WITH THE FUNDING SOURCE TO SHOW WHAT iLEVEL ALLOWS YOU TO ACHIEVE.
